By integrating behavioral health services directly into the primary care setting, patients can receive support for their mental and emotional well-being as part of their overall healthcare. This collaborative approach allows our medical and behavioral health teams to work together to develop care plans tailored to each patient's individual needs.

For Youths & Adults
Collaborative Care (CoCM) requires a team of providers. Trained Primary Care Clinicians (PCC) work with embedded Behavioral Health Care Managers (BHCM) to provide evidence-based medication or psychosocial treatments.

The Role of the Primary Care Clinician
