What is Chronic Care Management (CCM)?
Chronic Care Management is a coordinated health care service designed to help patients with two or more chronic conditions(such as diabetes, hypertension, or heart disease) manage their health more effectively outside of regular doctor visits. Key features of a CCM program include:

CCM programs aim to improve health outcomes, reduce hospitalizations, and enhance quality of life for patients with long term health needs. These services are often covered by Medicare.

NOTICE OF SPECIAL MEETING
OF
PUBLIC HOSPITAL DISTRICT NO. 3
OF GRANT COUNTY, WASHINGTON
The Board of Commissioners of Public Hospital District No. 3 of Grant
County, Washington has scheduled a Special Meeting on November 4, 2025
beginning at 5:35pm at Columbia Basin Hospital, Ephrata, Washington. The
District’s 2026 proposed budget will be discussed. Provisions are being
made to allow the public to attend this meeting by electronic means. Details, including a call-in number will be posted on the Districts web site:
https://www.columbiabasinhospital.org/. At that time and place the
Commission will consider any and all Hospital District business which may
come before it. This Notice is given pursuant to RCW 42.30.075.
