What is Chronic Care Management (CCM)?
Chronic Care Management is a coordinated health care service designed to help patients with two or more chronic conditions(such as diabetes, hypertension, or heart disease) manage their health more effectively outside of regular doctor visits. Key features of a CCM program include:

Coordination between multiple health care providers

Support with medication management

Home visits

And much more!
CCM programs aim to improve health outcomes, reduce hospitalizations, and enhance quality of life for patients with long term health needs. These services are often covered by Medicare.

The Board of Commissioners of Public Hospital District No. 3, Grant
County, Washington will be attending their annual Holiday Gathering in
place of their Regular Meeting.
The gathering will be held at the Millie’s,
114 1st Ave NW, Unit 106, Ephrata Washington on December 8, 2025,
beginning at 6:00pm
