JOB SUMMARY

The Clinic Referral Coordinator provides a variety of receptionist and skilled clerical duties in support of the Clinic physicians, Community Health Worker, Care Coordinator and Clinic Admin. Supervisor. This position is responsible for obtaining documentation and coordinating aspects of patient care to ensure continuity of care, with internal and external patients. Position performs prior authorizations for all imaging for Clinic and Hospital Providers.

DUTIES AND ESSENTIAL JOB FUNCTIONS

  1. Provides all necessary communications from the Clinic (patient letters; forms; documentation; customer satisfaction; business meetings; etc.).
  2. Works collaboratively with families, departments providing services, physician offices, and physician specialists to coordinate resources needed.
  3. Provides secretarial support to Physicians, Nurse Practitioners, Clinic Admin. Supervisor, and Nurses as needed.
  4. Confirms provider availability for cases, copies and distributes schedules, acts as a resource person within practice for other outpatient and inpatient departments.
  5. Coordinates, sends and tracks all outgoing referrals to other specialties and/or facilities.
  6. Obtains prior authorizations for imaging requests from CBFM and Hospital Providers.
  7. Assists Clinic Supervisor with any special projects or other as needed duties.
  8. Works with Community Health Worker and Care Coordinator to assure accuracy of community resource list and information.

POSITION QUALIFICATIONS

  • High school diploma or equivalent education is required.
  • Basic computer skills are required.
  • Knowledge of office procedures and functions including calculator and basic math skills are preferred.
  • Knowledge of basic office equipment including computer, copy machine, fax machine, and telephone system is preferred.
  • Previous experience in a medical setting and basic understanding of medical terminology and medical insurances is desirable.
  • Must have exceptional customer service skills, organizational skills, be self-motivated, and multi-task oriented.
  • Must be able to communicate with patients and co-workers in a caring, courteous, and professional manner.
  • Must be able to speak, read, write, follow oral/written instructions in English.
  • Bilingual Russian or Spanish preferred.
