If you are interested in signing up to be on our vaccination waiting list, please answer the questions below before emailing us.
Are you a resident of Grant County?
Due to the limited number of vaccine allocation to our clinic, we are prioritizing our full time residents within our service area. If you do not live within our service area please do not register with us at this time.
Are you eligible to receive the vaccine yet?

If you are eligible, or if you have any questions regarding the vaccination please email us your full name and a phone number where we may contact you, at COVID@columbiabasinhospital.org.
After you have emailed us the information needed to be placed on our vaccination list, a Columbia Basin Family Medicine representative will contact you to schedule an appointment for you to receive the first of the two-dose vaccine as vaccinations come available. Please be patient with us. Vaccines are provided to us based on our immediate service area.
The day of your vaccination appointment, please wear a mask and be prepared to be screened for COVID-19 symptoms when you arrive.
